  Adult Orthodontic Update: Esthetic Alternatives to Braces

A recent spurt in orthodontic technology, mainly generated by computer and Internet development, has led to a number of astounding advances in the area of esthetic orthodontics. Most adults remember the braces of their youth as the dreaded "train tracks" or "metal mouths." That, however, is history. Very few of my adult patients wear traditional metal braces. Those who are seeking an esthetic alternative to traditional braces can now choose from ceramic braces, clear aligners ("Invisalign"), or lingual (on the inside of the teeth) braces.

While all of the esthetic options are theoretically options for adults, the choice of appliances for any specific patient will be determined by the type of treatment that is needed. At the consultation, I review the patient's needs with them and inform them of their options. This is a highly individualized process to insure that each patient receives the highest quality treatment possible, and finishes with an excellent result that is both esthetically beautiful and functionally stable.

The technology behind some of these esthetic appliances is absolutely astounding. For example, lingual braces are made from a computerized model of the patient's mouth. The computer then generates customized wax templates which are then individually cast into gold. Each bracket is literally customized for each tooth. The wires for these braces are then bent by a robot to be customized for the shape of the patient's mouth.

Another appliance which is fascinating is Invisalign®. This also begins with a scan of the patient's teeth into a computer. The computer then virtually moves the teeth into alignment, and creates a clear template, or "aligner" to match every stage of this movement. Each one of these "aligners" is then worn for two weeks and slowly moves the teeth into place.

Many people ask me what the age cut-off is for orthodontic treatment. The answer is that as long as the underlying structures (gums and bone) are healthy, orthodontic treatment can be successful at any age. I feel blessed to be practicing orthodontics in this age of innovation, and I welcome patients of all ages to come and see the amazing changes that we can make in their smile.
